Los Angeles Daily News, NOTICE OF PROPOSED TITLE V PERMIT REVISION The South Coast Air Quality Management District (SCAQMD) is the air pollution control agency for all of Orange County and portions of Los Angeles, Riverside and San Bernardino Counties. Rule 3006 requires that the SCAQMD publish this notice prior to the revision of a Title V permit. Los Angeles Daily News, NOTICE OF PROPOSED TITLE V RENEWAL PERMIT The South Coast Air Quality Management District (SCAQMD) is proposing to renew the existing Title V permit that was previously issued to the following facility: Lifoam Industries LLC 2340 E. 52nd Street Vernon, CA 90058 Facility ID# 144455 Contact Person: Van Luong Truong Acting Plant Manager Lifoam Industries, LLC. 2340 E. 52nd Street Vernon, CA 90058 This is an existing facility applying for a renewal to their Title V permit. This facility is engaged in manufacturing expanded polystyrene (EPS) foam products such as small coolers, food containers, packaging material, and various custom molded products. This facility operates one polystyrene foam expansion/molding system and one boiler. The polystyrene foam expansion/molding system consists of two pre-expanders, 50 aging silos, 19 shape molding presses, and associated conveyors and hoppers. The VOC emissions from the polystyrene foam expansion/molding system are controlled by a regenerative thermal oxidizer. Pursuant to Title V of the federal Clean Air Act and SCAQMD Rule 3004(f), a Title V permit shall expire five years from the date of issuance unless such permit has been renewed. On Friday, February 27, 2009, at approximately 9:31 a.m., a 48-year old tamale vendor, Cosme Gonzalez, was shot and killed in what LAPD reports was an attempted robbery. Cosme leaves behind a wife and three young children. LAPD has issued a warrant for the arrest of one of the suspects, Werner Francisco, who is believed to have fled the United States to Mexico or South America. LAPD is still searching for the suspect(s) and encourages witnesses to come forward, even anonymously, to assist them in their investigation. The person or persons responsible for this crime represent an ongoing threat to the safety of the people of Los Angeles. Each bid must conform and be responsive to all pertinent Project Documents, including, but not limited to, the Information for Bidders. As security for its Bid, each bidder shall provide with its Bid form either (1) a bid bond issued by an admitted surety insurer on the form provided by the District, (2) cash, or (3) a cashier's check or a certified check, drawn to the order of the District, in the amount of ten percent (10%) of the total bid price. This bid security shall be a guarantee that the Bidder shall, within the time frame required in the Project Documents, enter into the contract to perform the Project. The successful Bidder shall be required to furnish a 100 % Performance Bond and a 100% Payment Bond if it is awarded the contract to perform the Project. The successful bidder and all of its subcontractors of all tiers shall register with the Department of Industrial Relations (DIR) as a contractor that is working on a public works project and shall pay all workers on all work on the Project not less than the general prevailing rate of per diem wages and the general prevailing rate for holiday and overtime work as determined by the DIR, State of California, for the type of work performed and the locality in which the work is to be performed within the boundaries of the District, pursuant to sections 1770 et seq. of the California Labor Code. Prevailing wage rates are available from the District or on the Internet at: http://www.dir.ca.gov. No contractor or subcontractor may be listed on a bid proposal for a public works project (submitted on or after March 1, 2015) unless registered with the Department of Industrial Relations pursuant to Labor Code section 1725.5 [with limited exceptions from this requirement for bid purposes only under Labor Code section 1771.1(a) in which circumstance the contractor or subcontractor is ineligible to respond to a bid or to do public work]. No contractor or subcontractor may be awarded a contract for public work on a public works project (awarded on or after April 1, 2015). This project is subject to compliance monitoring and enforcement by the Department of Industrial Relations. The District will conduct a mandatory informational pre-bid conference relative to the projects at which time the bid package will be handed out and discussed with all pre-qualified contractors interested in bidding. The location of the conference shall be held at the Facilities Document Office, located at 4029 N. Las Virgenes Road, Calabasas Ca. 91302 behind AE Wright Middle School. The pre-bid conference will commence promptly at 10:00 A.M. on Wednesday January 20, 2016. the District requires that each bidder possess the following classification(s) of contractor's license(s) at the time the bid is submitted: Bid Package Description License License HVAC "Early Retirement" Warm-air Heating, Ventilating C-20 Replacement Projects & Air Conditioning The Bidder's license(s) must remain active and in good standing at the time of the bid opening and throughout the term of the Agreement. The Project Documents include the District equipment and program specifications and are pursuant to the requirements of the Southern California Edison Early Retirement Program for which bidders are to base their bids. Substitutions: Any request for substitution of equipment and/or material must be submitted no later than 10 (ten) days after the pre-bid conference for evaluation and approval. A full list of proposed project equipment shall be attached with the contract documents and shall be discussed during the mandatory pre-bid conference.
